Our patient portal provides secure online access to your medical records and offers an easy and convenient way to communicate with Colbert Family Health & Wellness.

Key features include:

• send and receive secure messages with our practice
• make changes to your personal information
• schedule appointments
• view previous and upcoming appointments
• view and print medications and allergies
• request prescription refills

Sending Messages

• to send a message more securely than email, click the Messages tab at the top of the screen
• each message you send goes directly to the practice
• you will receive email notification when a response is available in your patient portal
• important medical records and documents will be saved in your patient portal

Scheduling and Viewing Appointments

• click on the Appointments tab at the top of the screen to schedule or view all appointments
• date, time, location and doctor information will be available for each scheduled appointment
• to schedule an appointment, click Schedule My Appt and enter the necessary information

Viewing Medications and Allergies

• click the Meds tab at the top of the screen to view all medications, date prescribed, how often to take it, and important drug information
• to request a refill, click the refill box next to the drug and select the pharmacy where you would like to it to be filled
• click the Allergies tab at the top of the screen to view all current allergy information

Updating Your Account Info

• click on the Account Info tab at the top of the screen
• you can make changes to phone numbers and address
• you can update insurance information
• you can update demographic information
• make changes conveniently at home and save time on the day of your appointment
